In a northern London home an evil awaits under the floor boards. Frank Cotton, previously an american thrill seeker purchased a mystical box at a middle eastern bazaar and attempted to use the box to taste pleasures outside the normal realm of human sensation. Unfortunately, for him, to do so meant unleashing a collection of demons (the cenobites) back unto earth...
Larry Cotton, his brother, comes to the house to stay with his demented wife Julia (who had formerly had a violent and passionate love affair with Frank) and his daughter Kirsty. Accidently Larry cuts his hand and the oozing blood that drips from his hand seeps into the floorboards onto the remains of his dismembered brother Frank. The blood begins to replenish the remnants of Frank, breathing new life into his evil carcass, making it hunger for more. Larry's twisted wife Julia begins to feed into her lovers needs and lures new victims for the creature that is Frank to feed upon and replenish the blood he is missing to become a man again. He knows he must flee because the Cenobites are searching for him to bring him back to hell were he belongs.
Kirsty gets trapped in the clutches of her disgusting uncle, fooled because he disguises himself in her fathers skin. Later once she has narrowly escaped, she becomes entranced in the box and begins to solve it, causing a parallel universe to open yet again. This calls upon Pinhead and the other Cenobites to our dimension. To save her life Kirsty attempts to make a deal with them, giving them Frank for her soul.

Kirsty now in a mental hospital, only hours later recoils at the thought of her dissipated family, the rotting skin of her fathers corpse, her murderous stepmother and also the perverse pleasures of the demonic Cenobites. She is sorrowfully only comforted by Dr. Channard, a psychiatrist who oddly, is very interested in her stories. He hears of the blood stained mattress that Kirsty needs to destroy in order to rid the world of Julia, her stepmother's, evil presence for once and forall. Unfortunately for Kirsty, Dr. Channard has obsessively devoted his life to solving the Lament Configuration, the key to the doorway of hell, and sees one step closer with Kirsty.
History repeats and yet again victims are lured to their untimely deaths as Dr. Channard brings patients to the blood soaked mattress conducting grisly human sacrifices to resurrect the body of Kirsty's evil stepmother Julia. She will once again walk among the living and help Dr. Channard to completing his life's goal by unlocking the Lament Configuration and releasing the unlimited horrors of ultimate pleasures of Hell.
Kirsty, disturbed by visions of her father, takes Tiffany, another patient, beyond the limits into Stygian corridors in an attempt to rescue her father from his eternal torment. Again she will have to confront and barely escape the clutches of Pinhead and the dark desires of the Cenobites, yet ultimately placing herself in the threshold of Leviathan - the Lord of Hell's Labyrinth..

The Marbled features of Pinhead are amongst the writhing figures and distorted faces etched into the surface of an intricately carved pillar bought by rich and spoiled nightclub owner J.P. Monroe.
When Joey Summerskill, a young ambitious television reporter, witnesses the bizarre death of a teenage boy torn apart by chains in a hospital emergency room, she discovers from the boy's trendy girlfriend, Terri, that the chains emerged from a small puzzle box which the boy stole from the pillar displayed in J.P.'s club.
While Joey and Terri investigate the origins of the puzzle box, J.P. accidentally spills a few drops of blood on the pillar, releasing the imprisoned soul of Pinhead. Finally freed from the pillar by J.P.'s hedonistic greed, Pinhead walks the earth again in an orgy of blood and desire, creating an army of new Cenobites from the transmutated flesh of his victims to reclaim the box and set him free from the powers of Hell forever.
Through video tape interviews with Kirsty Cotton recovered from the Channard Institute, joey and Terri learn about the demonic Cenobites and the power of the Lament Configuration box, the only mean of sending Pinhead back into the Outer Darkness.
Joey, whose dreams are tortured by recurring visions of her dead father, and Terri, who would give anything to dream at all, are all that stand in Pinhead's way; and before the battle between good and evil reaches a final, incredible confrontation, dreams become reality as they receive help from a very unexpected source...

It's the year 2127. Pinhead, the evil cenobite of the series, has found himself on board a space station in outer space, run by scientist Dr. Merchant. He has created the ultimate space station for a special purpose: to destroy the Lament Configuration box, which has brought silent shame to his family for centuries. His ancestor, Phillip L'Merchant, a French toy maker in the 18th century, built the evil puzzle box that unleashes the horrors of Hell. It was used by a magician of the Dark Arts named de L'Isle to create a beautiful demon named Angelique.
As the generations passed, his ancestors began the work to create a box that would destroy the Lament Configuration, until 1996, when one ancestor, John nearly fell to the power of the box through Angelique. Now Dr. Merchant, the latest in the bloodline, has created a new box which will shut the gates of Hell forever.

Kirsty's back again and this time with a husband. Kirsty and Trevor start off by driving off of a bridge, their car plunges into the river below them. Trevor manages to escape with his life but, even though both doors are found to be open by police divers, there is no sign of Kirsty and to the casual observer Trevor doesn't seem too heartbroken about the whole accident.
Trevor, now alone, is plagued with blinding headaches and blackouts. People close to him begin dying one by one. It flashes from one dream to another, one hallucination to another and Trevor can never quite figure out what the hell is going on here. Is Trevor dreaming or is it real?
The Lament Configuration box has been solved again and the demonic Pinhead has returned to Earth with his legion of Cenobites to bring torture and death to those who stand in his way and to release Hell on Earth once more. But Kirsty Cotton, the only person who has defeated Pinhead twice, is back and ready to fight Pinhead and the Cenobites. The Cenobites offer up all the pain and madness their Hell has to give and Kirsty has a chance to face off Pinhead once again.

A female reporter Amy Klein (Kari Wuhrer) for a London newspaper is sent to Bucharest to investigate an underground suicide cult (the Deaders) who can bring back the dead. She is quickly drawn into their dark world and soon can see no way out other than to join them. The Cult is led by Winter (Paul Rhys). At the address given to her by her editor, Amy finds the corpse of a girl and a puzzle box. In her hotel room, despite being told not to do so by Marla (the corpse), she opens the puzzle box. Hooks appear which try to pull her into the box. Pinhead the Cenobite (Doug Bradley) appears, warning her to not think she is in no danger. Marla tells Amy to meet Joey (Marc Warren) by asking for him on the last train in the Bucharest subway. Marla tells Amy that only she can help them. Joey tries to dissuade her from her quest but Amy doesn't relent. On reaching the "Home" of the cult she witnesses a man being brought back from death. There Winter tries to influence her. Slowly she gets tangled to an extent that she's inches away from her death. But Cenobites appear and tear apart Winter. They accuse him of invading their world. They also try to claim her but instead of giving her soul to them she chooses to kill herself. In the end of the movie the editor Charles (Simon Kunz) is shown to entice another reporter onto presumably another mystery. Also the puzzle box has been shown to be safe and intact despite the explosion which brought down the whole building.
